Request Restrictions: You may request limits on PHI use for TPO. We are not required to agree but will comply if feasible, except in emergencies. Submit requests in writing.

Confidential Communications: You may request alternative contact methods (e.g., email only). Submit requests in writing; we will accommodate reasonable requests.

Amend PHI: You may request corrections to inaccurate PHI. Only we, as the creator of your PHI, can amend it. Submit requests in writing.

Accounting of Disclosures: You may request a list of non-TPO disclosures made in the past 6 years (not before October 1, 2025). Submit requests in writing; we may charge for photocopying.

Paper Copy of This Notice: You may request a paper copy at any time, even if you access it online.

Changes to This Notice

We may revise this notice, effective for existing and future PHI. Updates will be posted at https://contact.renewwound.com/hipaacompliance with the new effective date.

Complaints

If you believe your privacy rights were violated, you may file a complaint with us or the U.S. Department of Health and Human Services. Contact our Privacy Officer at the details below. You will not face penalties for filing a complaint. All complaints must be in writing.

Other Uses of PHI

Uses or disclosures not covered here or by law require your written authorization. You may revoke authorization in writing, but prior disclosures cannot be undone. We retain records as required by law.
